AVIATION EXPLORER POST 9 FORMS

The following forms and information documents are used to inform both the Explorers and their parents about the Post activities for the coming year and to register both new and returning members. Each member, either new or current, needs to fill out these forms, have them signed by their parents or guardians, and submit them to the Post Chairman at the start of each new school year.

This is the information that outlines our proposed Official Post Flying activities for the coming year. It also lists both the aircraft and pilots that will be flying the Explorers during these flights. While we have a very aggressive flying plan, changes can and will occur during the year due to both weather conditions and aircraft availability.

This is the Parental Consent Form that each Explorer must have filled out and signed by both parents or guardians before he or she can take part in any Official Post Flying Activities. A new Consent Form must be filled out and submitted each year.

This is the General Parental Consent Form that each Explorer must have filled out and signed by both parents or guardians before he or she can take part in any Learning For Life Explorer Post Activities. A new Consent Form must be filled out and submitted each year. This form is sent to the local Learning For Life Office to register the Explorer as a post member.

This is the LFL General Information Form that each new member must have filled out and signed by a parent or guardian in order to initially join an Explorer Post. [This form only needs to be filled out when the student first joins the Post, returning Post members do not need to fill out this form.]

This is a simple contact form that is used by the Post to update our Roster information on each active member. A new copy must be submitted each year to keep info such as home address, cell phone numbers, and E-Mail addresses up to date.

This is a simple contact form that is used by the Post to update our Roster information with current contact info for the Parents of each active member. A new copy must be submitted each year to keep info such as home address, work and cell phone numbers, and E-Mail addresses up to date.
[FORM] Aviation Explorer Post 9 Additional Activity Release Form for 2010
This is a Parental Consent Form that each Explorer must have filled out and signed by both parents or guardians before he or she can take part in any Post Activities that are outside of the Official Fly Along Plan. Occasionally we may offer other opportunities to the Explorers to participate in events that are not covered by the LFL program and it insurance coverage. These events are optional and Explorers cannot participate unless their parents or guardians have read and signed this release form.

GENERAL POST FORMS

One of the following Tour Permit forms must be filled out and submitted to the local LFL office by each Post prior to an event that requires the Post to travel as a group.

AIRVENTURE 2010 AEB FORMS (for Explorers & Parents)

Each Aviation Explorer must download the following forms, fill them out, and return them to the Post Advisors before the deadline, in order to be eligible to attend AirVenture 2010 as part of the Aviation Explorer Base.

This is the information that explains what we do at AirVenture and gives both Explorers, and their Parents, the info they will need to attend.

This is the Individual Registration Form that each Explorer must fill out to attend.

AEB 2010 Base Camp Policies
Each Explorer must download and review these Base Camp Policies in order to attend.
AEB 2010 Medical Form
As part of the requirements of the Aviation Explorer Program (and the BSA), each person that attends AirVenture as part of the AEB (both Explorers and Advisors) must have this Health Form filled out and signed by their physician. It will be kept on file at the AEB Base Camp during the week in case of a medical emergency.
